    Dr. Peterson and other members of this group have worked for a decade now to advance and support distributed and federated database models which can support practice-based research. The work has not been easy, at least in the US. The model outlined in this manuscript indicates some of the complexity involved in meeting the needs of all actors in the PBRN world. This model will serve the primary care PBRN informatics world well as we continue to push a federated approach to gathering and using electronic data to improve clinical care and research across multiple independent organizations. There are several competing movements currently. One which wants to solve the data sharing and inter-related trust issues through administrative means and move each organization's clinical data to a central site. This model is unlikely to appeal to large numbers of independent, unrelated and at times competative organizations who are willing to ask and answer questions together but not to co-mingle their data for researchers and others to mine over and over. The second model standardizes data at local sites but then conducts the analytics at each site. This works for large sites but is not logical when many of the organizations consist of small practices without analytical support or analytical software. This paper will help us understand where, how and why the central datawarehouse and distributed analytical models will not serve our needs. This will be important as the systems that will serve are needs are more complex and will take time to develop.
