Assessment
    HbA1c test in the last 6 months
    LDL-cholesterol test in the last 12 months
    Microalbumin test in the last 12 months
    BP check at every visit
Treatment
    HbA1c ≤ 8%, or >8% and on a hypoglycemic agent
    LDL-cholesterol ≤ 100 mg/dL, or >100 mg/dL and on a lipid- lowering agent
    BP ≤ 130/85 mm Hg, or >130/85 mm Hg and on an antihypertensive agent
    Microalbumin in urine >30 mg in 24 hr and on an ACE inhibitor or ARB
Target attainment
    HbA1c ≤ 7%
    LDL-cholesterol ≤ 100 mg/dL
    BP ≤ 130/85 mm Hg
