| Notes: Cox regression analysis was used to compare the groups. P values are adjusted for group-randomized trials according to the method of Murray.18 The intraclass correlation coefficient (ICC) was 0.12 for all deaths, 0.14 for coronary heart disease deaths, and 0.14 for cardiovascular deaths.
