LINKING
Annals of Family Medicine, Inc., has no objection to links from other websites to content on the Annals website as long as the following guidelines are followed:
- Annals content may not be downloaded to another Web server, displayed or reproduced by any other means in any other format.
- Annals content may not be framed, 'locked' or 'trapped' within any borders, banners or other visual images other than those placed by the Annals of Family Medicine, Inc., itself.
- Links should be external ones in that the Annal's URL is visible in the address window.
- Plain text should be used to indicate the link. Use of the Annal's logo is prohibited without written consent from the Annals of Family Medicine, Inc.
- The requester should confirm the link by providing the Annals of Family Medicine, Inc., with the requester's URL and location of the link to the Annals content for the Annals of Family Medicine, Inc.'s file tracking purposes.
- Links should not be presented in such a way it may, can, or is perceived as a sign of the Annals of Family Medicine, Inc.'s endorsement of any company, product or service.

Educational Use
Content on this site is copyrighted. You may print materials from the Annals website for non-profit, educational purposes only, under the terms and conditions of the definition of 'fair use.' Proper attribution, including a notice of copyright, must be included on all copies.
Abiding by the principal factors of fair use, one can utilize a portion of a copyrighted work "as is" for purposes of non-profit research and education. Appropriate credit of the source is required. The use of embedded third-party copyrighted works, or portions thereof, for any other purpose is not deemed fair use.
Annals of Family Medicine is registered with the Copyright Clearance Center (CCC) for academic classroom use. Please submit your request to the CCC Publication Department online at http://www.copyright.com.

- Identify the specific content (for instance: entire article, figure 1, table 3)
- Provide a sample of the original 'work'
About your proposed use:
- Information regarding your publication, program, or product
- URL of website, if applicable
- Name of publisher or sponsor
- Type/format of publication in which Annals content will appear (for instance: website, newsletter, book, periodical, CD-ROM)
- Estimated number of copies to be printed or produced for distribution
- Whether the copies will be sold or free. If free, what funding and/or sponsorships, if any, are affiliated with the publication/ program/ project? If a presentation/ workshop/ lecture, is there a registration fee for participants?
Please note: Responses to permission requests usually take 10 to 14 business days from the time a request is received. Response times may be longer during times of high volume. Incomplete requests will be returned and will not be processed.
Fees and Payments
A permission fee is charged when material is to be used in another publication (e.g., newsletter, textbook, manual, journal, magazine, etc.) or if there are fees attached to the content distribution. Permission fees will depend upon the amount of content being reprinted. Payment should be made in U.S. currency and must be received before permission is granted. (Credit cards currently are not accepted.)

To created single reprints for personal use, print the Adobe Acrobat (PDF) files available on the individual article pages.
