    This editorial and the other articles in this issue of AFM are refreshing in bringing a new generation of leaders into the disucssion of the future of family medicine. While a diversity of career options and team based care are embraced, what I find missing is the actual transformation of how care is delivered and the role of the patient. This editorial begins promising by mentioning internet applications of communication and care but does not describe how they fundamentally change practice. Qliance in Seattle (Erika Bliss is second author here), Greenfield Health in Portland, and Eisenhower Primary Care 365 in La Quinta, CA are just a few examples where a new platform of communication and care to a great degree replace episodic brief office visits. When office visits happen, enough time is available to take a deep dive into the patient's health. Patients, empowered by having their own records and guidelines for care, are the new practitioner as they will be increasingly able to care for themselves with the family medicine team in a role of coach, advisor and motivational counselor for prevention and chronic illness management, the two dominant areas for family medicine. Let's explore real change in how we work and not focus so much on how a new generation needs greater flexibility and diversity in the workplace, which are important. Family medicine is a service industry and how we do that in the 21st century is fundamentally different than the past.
