    Bellin and Dr. Jerzak are clear leaders in the field of practice re- design and transformation. I've had opportunity to spend time with and learn from him and his team over the past few years. We have a very similar model at Cleveland Clinic and have been sharing a lot of ideas and experiences with the Bellin team. Physicians can't do it alone. Team-based care is a proven way to move towards population management while also helping prevent and mitigate Caregiver burnout.

The principles and concepts outlined in the supplemental appendix show the way forward for other organizations looking to implement team based care. The principles and concepts outlined in the supplemental appendix show the way forward for other organizations looking to implement team based care. You can see Dr. Jerzak, Dr. Robert Mead and other members of the Bellin team in action by viewing video portions of Steps Forward Modules: "Team Documentation", "Previsit Lab" and "Synchronized Prescription Renewal". The Steps Forward website provides these and many other practical modules which can help guide physicians who are in search of a more efficient,joyful team-based model of providing patient care.

    While Drs. Bodenheimer and Sinsky have written about how achieving the triple aim is possible only by adding the fourth leg of provider satisfaction. I would add that it is not just the providers who are feeling the effects of burnout, but our support staffs as well. In our model we have seen improvement in the professional satisfaction of all the members of our care teams. A natural consequence in this has been an almo...

    While Drs. Bodenheimer and Sinsky have written about how achieving the triple aim is possible only by adding the fourth leg of provider satisfaction. I would add that it is not just the providers who are feeling the effects of burnout, but our support staffs as well. In our model we have seen improvement in the professional satisfaction of all the members of our care teams. A natural consequence in this has been an almost across the board improvement in our ambulatory quality measures. As the Medical Director for Population Health at Bellin, I am responsible for achievement of these quality measures. I am seeing an engagement in quality improvement from not only our providers, but our staff as well, and at a level that we have not seen previously.

    As Dr. Jerzak mentioned in his paper, our staff now feel empowered to help improve the health of our patients and are taking an active role in our improvement activities. The office visit activities have shifted from a "check the box" mentality, to "How are we going to help our patient today? I truly believe that triple aim outcomes will be realized with our model.
