    I couldn't agree more with the conclusions drawn in this article and with Dr. Wasson's comments. I've had the pleasure of watching Dr. Ho set the bar for primary care physicians in the state of RI and challenge the belief that scarce resources prevent us from meeting the unique needs of each patient. She has also been a standout in terms of making use of CurrentCare, RI's statewide health information exchange, and lever...

I've had the pleasure of watching Dr. Ho set the bar for primary care physicians in the state of RI and challenge the belief that scarce resources prevent us from meeting the unique needs of each patient.     Over many years Drs. Ho, Antonucci and their colleagues have demonstrated that by really listening to their patients (in a standardized way) and by focusing on the basics (such as continuity and access) primary care practice will be of high value for patients and the clinicians who serve them. (1,2)

    In their current article (Using Patient-Entered Data to Supercharge Self-Management) they bring attention to a free website (www.HowsYourHealth.org) that evolves in response to the input of clinicians like them to make better care possible. The newest iteration of the website greatly simplifies assessment and management of patients with chronic conditions (1,3) and provides patient-reported measures that, by the mere push of a button, measures the foundations that hold up the medical home.
