    The original version of this article contained an error. In the penultimate paragraph, the phrase, "Hemoglobin A1c >= have..." should have read, "Hemoglobin A1c >=9 have... ." The error has been corrected in the version linked here on the Highwire site, but had already gone to press before discovery. The online version therefore differs from the print. A correction will be published in the July/August issue. We regret the error.
