The authors' synopsis of their findings: "The experience of the ePCRN project has been one of technical innovation constrained by market limitations" likely could have been anticipated prior to initiating their project. An interim solution, that we have pursued since 1995 in PPRNet is to work with one EHR vendor and their primary care practice clients to build a research and learning network.

    Dear Authors, Hello Richard

Congratulations on a great paper and a groundbreaking report. We, in Israel are very fortunate to have an integrative system on top of our practice EHR software as well as a data-mining additional layer. Thus, we are able, on a routine basis to pose queries and get good quality data for multiple clinically generated questions. A month ago our MOH contracted with a vendor to have the whole Healthcare system connected by the integrative system. We expect this to be accomplished by the end of the year. An example of such research can be found in:

1.Shuval K, Berkovits E, Netzer D, Hekselman I, Linn S, Brezis M, Reis S. Evaluating the impact of an evidence-based medicine educational intervention on primary care doctors' attitudes, knowledge and clinical behaviour: a controlled trial and before and after study. J Eval Clin Pract. 2007 Aug;13(4):581-98. PubMed PMID: 17683300.

2.Shuval K, Linn S, Brezis M, Shadmi E, Green ML, Reis S. Association between primary care physicians' evidence-based medicine knowledge and quality of care.Int J Qual Health Care. 2010 Feb;22(1):16-23. Epub 2009 Dec 1. PubMed PMID: 19951965.

I estimate the number of studies that have used this capacity around a hundred by now. It will be great to share experiences, best practices and thoughts on how to proceed within and outside national and systems boundaries. Again, great job

    Best,

    Shmuel
