    This is a remarkable national study of patient-reported care experiences within federally qualified community health centers. I am most intrigued by the consistent and strong association between patient-centered communication with members of the health care team, and all measures of perceived quality. Current standards for the PCMH emphasis more structural elements of the medical home, the "anatomy." Developing high-functioning team-based care with a focus on patient-centerd communication may be our next challenge for the Medical Home, the "physiology" of truly delivering patient-centered care.

However, from the patient's point of view, they assume (correctly or not) that the "structure" of the home is sound and functioning well. Patient-centered communication and access to care are two areas where the patients are the best judge of quality.
